Tightening the screws on pricey imaging exams, health insurer Anthem will no longer allow many patients to get MRI or CT scans at hospital-owned outpatient facilities, requiring them to use independent imaging centers instead. The insurer began phasing in these changes in July and expects to finish by March.

Anthem says the change is aimed at providing high-quality, safe care while reducing medical costs.

Anthem Blue Cross and Blue Shield officials said on Wednesday the company will no longer offer health insurance plans in 61 counties in Kentucky in 2018.

Kentuckians in more than half of all counties who buy insurance through HealthCare.gov next year will have a much more limited choice of doctors and hospitals.

That’s because the only insurer left in Kentucky offering exchange plans in all 120 counties — Anthem Blue Cross Blue Shield — will only offer an HMO plan in 74 of those counties starting Jan. 1.
